At one time or another, almost everyone experiences a ringing in their ears. It’s common after sitting through a loud concert or an exuberant football game, or after taking aspirin or an antibiotic for a while. But as many as 50 million Americans have chronic tinnitus (pronounced tih-NITE-us or TIN-ih-tus)—a constant ringing, whistling, buzzing, chirping, hissing, humming, roaring, or even shrieking. (If you don’t have tinnitus, you can get an idea of what people with the condition hear at the American Tinnitus Association’s Web site.)
Chronic tinnitus can be caused by a variety of things, from impacted ear wax to medications that damage nerves in the ear, middle ear infection, and even aging. Damage to hair cells in the ear’s cochlea (see the illustration below) are suspected as a common pathway for these causes. As I write in the September issue of the Harvard Women’s Health Watch, chronic tinnitus can also be a symptom of Ménière’s disease, a disorder of the balance mechanism in the inner ear. Most people are pretty good judges of what’s going on with their own bodies. But telling a heart attack from other causes of chest pain is tough stuff—even, it turns out, for highly trained doctors. That’s why I thought this personal story, written by a Harvard doctor who has heart disease, would make an interesting read. It’s an excerpt—the full version can be found in Heart Disease: A guide to preventing and treating coronary artery disease, an updated Special Health Report from Harvard Medical School.
Early one spring, I noticed a burning sensation high in my abdomen whenever I walked up a hill or worked out on the treadmill. I felt perfectly healthy otherwise. I had lots of energy and could do high-level exercise on the treadmill—once the burning sensation went away—without becoming short of breath. I thought it was just heartburn, so I started taking powerful acid-suppressing pills. They didn’t help.

Back in June, federal authorities unveiled MyPlate, an icon designed to help Americans follow healthy eating patterns. It’s a nice, colorful image that was a welcome successor to the misguided MyPyramid. But it doesn’t offer much in the way of useful information.
A group of my colleagues at Harvard Health Publications worked with nutrition experts at the Harvard School of Public Health to create a better version. We call it the Healthy Eating Plate. “We gave MyPlate a makeover to provide consumers with an easy to use but specific guide to healthy eating based on the best science available,” says Dr. Anthony Komaroff, a professor of medicine at Harvard Medical School and Editor in Chief of Harvard Health Publications.
The Healthy Eating Plate uses visual elements of MyPlate as a starting point, because the government’s icon is already becoming a recognized teaching tool. But the resemblance ends there: Read more »

I often think of the well-known expression “perfect is the enemy of good” when I am endlessly rewriting an article to make it better (when it is already good enough) and in the process just make or even miss a deadline. But this old saying also reflects the dark view many people have of perfectionism. As a personality trait, it is seen as obsessive and at times pathological. People who are perfectionists may become so focused on setting a high standard for themselves that they live their lives as if graded constantly on a report card.
But perfectionism has a bright side, too. Desirable aspects of this personality trait include conscientiousness, endurance, satisfaction with life, and the ability to cope with adversity. This helps explain why some perfectionists become corporate leaders, skilled surgeons, or Olympic champions.
Dr. Jeff Szymanski, a clinical instructor of psychology at Harvard Medical School and executive director of the International OCD Foundation, believes it is possible to become a better perfectionist—by building on the strengths of this quality and learning to minimize its drawbacks. It isn’t easy to get rid of a harmful habit like drinking too much, or to make healthy changes like losing weight and exercising more. Media stories about people who run marathons a year after surgery to bypass cholesterol-clogged arteries or who climb Mt. McKinley after being diagnosed with diabetes are interesting, but they don’t resonate with me. Mostly it’s because they often leave out the hard work needed to change and the backtracking that invariably accompanies it.
I ran across a truly inspiring story the other day in the American Journal of Health Promotion—one that shows how most of us ultimately manage to make changes that improve our lives. The journal’s founder and editor, Michael P. O’Donnell, wrote a moving essay about his father, Kevin O’Donnell. Once an overweight workaholic who smoked and drank heavily, ate mostly meat and potatoes, and didn’t exercise—and who eventually needed a double bypass—Kevin O’Donnell gradually made changes to improve his health. Now, at age 85, he has the cardiovascular system of a 65 year old and is working on a house-building project in North Korea.
