The April 27, 2011 issue of Science Translational Medicine included a study titled “Differential Metabolic Impact of Gastric Bypass Surgery Versus Dietary Intervention in Obese Diabetic Subjects Despite Identical Weight Loss.”
Melissa Bagloo, MD, Assistant Professor of Clinical Surgery at the Center for Metabolic and Weight Loss Surgery, NYP/Columbia, explains the context and importance of this study.
Q: What did this study find?
Dr. Bagloo: For years, surgeons have observed that gastric bypass surgery cures diabetes in over 80% of patients with diabetes. This improvement in blood sugar levels happens almost immediately after surgery, and far before any significant weight loss occurs. What’s more, studies have found that when patients lose the same amount of weight through diet as other patients lose after surgery, those who had surgery experience significantly better improvement in their diabetes than those who lost weight non-surgically. So we know surgery dramatically improves or resolves diabetes, but we do not know why this happens.

September will mark 25 years for me with type 1 diabetes, but I still haven’t learned that an afternoon of lazy 200+ mg/dl’s that won’t budge, even after multiple boluses (and one really solid rage bolus where I actually grunted “You. Frigging. Diabetes.” as my fingers mashed the buttons), after repeated tests that showed climbing numbers … wouldn’t you think I’d inspect that infusion set? Maybe just give it a peek? See how things are doing there, on the back of my hip, where that 6 mm cannula is resting (hopefully) comfortably?
Oh, you mean I shouldn’t have waited until I smelled that distinct scent? The one that smells like a cross between bandaids and the dentist’s office? And then, when I dabbed at the gauze patch around my site and felt the dampness, I still didn’t really hone in on it because I was so high that everything was on like a 20 minute delay? Read more »

After spending an entire vacation reading stories, I would like to start tonight’s post with a tiny dose of fantasy. Can we try using a daydream to learn something about the challenge of making good nutrition choices?
The fantasy goes something like this…
You have just been sentenced to eternal life on a far-away sun-drenched island. This island has mountains, paved roads, wide bike lanes, and mountain bike trails. You get to take two bikes, a couple riding buddies and your family—if they’ll go. You also get to take one Apple product.
Sounds good so far.
The kicker is that you only get four food choices—and liquids count.
You are a cyclist, so after coffee and beer there are only two food choices remaining. Obviously, you will need a protein source. Smart choices here would include nuts, mercury-free fish or organically-fed animals. The protein isn’t the point, let’s keep moving.
Now we are down to the carbohydrate source.
Choose one of the following:
A.) Arugula
B.) Quinoa
C.) Cranberries
D.) Fruit Loops
Herein lies the primary hurdle that smart-nutrition advocates face: unhealthy simple sugars taste really good. Read more »

Oh rotting, feeble pancreas of mine,
Won’t you be my Valentine?
Won’t you wake from your long sleep
And make some insulin, you creep?
What makes you sit, all shaped like a wiener,
Lazy and dull, with a pompous demeanor?
What makes it okay, that for your enjoyment
You’ve spent twenty plus years filing unemployment?
We need to start over; we need to be friends.
We need this whole type 1 diabetes to end.
I’m tired of shots and I’m sick of the lows,
So I think we should talk about ending this row.
I could use a break, my corn-cob-shaped friend.
I’d love to have “old age” listed as my end.
I think that your time off has drawn to a close.
I’d like working islets, and plenty of those.
How ’bout it, old pal? Care to start working?
Care to start minding duties you’ve been shirking?
I promise to be an attentive best friend,
I’ll thank you each morning and as the day ends.
I won’t take for granted the hormone you make
And I’ll forgive you for the last 24 years’ mistake.
I’ve brought you some flowers and a Border’s gift card,
In hopes that when I bring milkshakes to the yard
You’ll be so inclined to jump start all those islets
Who’ve been holding their breath for so long that they’re violet.
So what do you say, oh pancreas of mine?

I have no idea how it happened, but yesterday was a crummy day, diabetes-wise. Somehow, early in the evening, I heard the Dexcom singing from the kitchen countertop, and BSparl and I went over to investigate.
“High.” With a long line at the very top of the Dexcom screen.
“Hi to you, jerkface,” I said, pulling out my meter to see just what the greeting was about. And I saw a sticky 451 mg/dl blinking back at me.
“What the fern?” I couldn’t figure out how I ended up so high, especially since after lunch I was 174 mg/dl and flatlined on the Dex.
And I was so angry. How does this happen? Did I eat the wrong thing? Take a shallow bolus? Is the pump ferning with me? Could the insulin have spoiled? Did I just lose track of everything and my numbers went berserk on me? Read more »
