Extra fat that accumulates around the abdomen goes by many names: beer belly, spare tire, love handles, apple shape, middle-age spread, and the more technical “abdominal obesity.” No matter what the name, it is the shape of risk.
Abdominal obesity increases the risk of heart attack, stroke, diabetes, erectile dysfunction, and other woes. The danger zone is a waist size above 40 inches for men and 35 inches for women.
As I describe in the April 2011 issue of the Harvard Men’s Health Watch, beer is not specifically responsible for a beer belly. What, then, is to blame? Calories. Take in more calories with food and drink than you burn up with exercise, and you’ll store the excess energy in fat cells.
Many studies indicate that people who store their extra fat around the midsection (apple shape) are at greater risk for heart and other problems than people who carry it around their thighs (pear shape). An analysis of 58 earlier studies covering over 220,000 men and women suggests that excess fat is harmful no matter where it ends up. This work was published in The Lancet. Read more »

A friend of mine is in great physical shape but her husband (we’ll call him “Mr. B”) has gained 40 pounds since they were married five years ago. He also has familial hypercholesterolemia, and several of his relatives have had heart attacks at young ages. Mrs. B is distraught – she is worried about her husband’s health, and has tried to gently nudge him towards healthier eating habits and regular exercise (as well as taking a statin for his cholesterol). Unfortunately, the nudges were received as nagging, and a wedge has formed between them in their relationship.
Last week my friend planned a trip to a primary care physician in the hopes that he would educate Mr. B about the dangers of being overweight and not treating his high cholesterol. “Surely Mr. B will listen to an expert” she thought, “then perhaps he’ll realize that he has to change his behavior.”
Unfortunately, the primary care physician didn’t offer any health counseling to Mr. B. Not only did he not mention that Mr. B should lose weight, but he didn’t provide any warnings about the dangers of untreated, very high cholesterol levels. He merely reported that Mr. B’s total cholesterol was 300, and that a statin was indicated.
Mrs. B was crestfallen. She was depending on the physician’s authoritative input to help her come up with a strategy to steer her husband towards better health. Now Mr. B was left with the impression that things were more-or-less ok, and that his wife’s concerns were exaggerated. Read more »

College-aged women in the UK say they would trade longevity for an ideal body weight.
320 women studying at 20 British universities (ages 18-65; average, 24.49) completed a survey in March.
The research, conducted for new eating disorder charity The Succeed Foundation, in partnership with the University of the West of England (UWE), found that nearly 30% of women would trade at least one year of their life to achieve their ideal body weight and shape:
–16% would trade 1 year of their life
–10% would trade 2-5 years
–2% would trade 6-10 years
–1% would trade 21 years or more Read more »

For competitive cyclists, Sunday morning usually signifies a time for combining spirituality with calorie-burning. Whether we are immersed in the total focus of a hotly-contested bike race or meditating our way through a seemingly endless training ride, it’s a given that most cyclists use Sundays to churn out the kilo-joules.
This kind of Sunday-behavior differs significantly from many regular (normal) people, who like to sleep late, get up slowly, dress themselves nicely and amble off to church. It goes without saying that this kind of spiritual exercise doesn’t burn many calories. And it is also well known that worship and consuming high-calorie comfort food frequently go hand in hand.
In the hard-to-believe-that-people-study-this kind-of-thing category, comes a report that frequent churchgoing in young adulthood increases the risk of obesity in middle age. Really, I am not making this up. The story was reported prominently here, on the theHeart.org. Read more »

For years I have avoided Medicare breast reductions for a number of reasons:
(1) Poor pay for hours of work. An average breast reduction when done to a high standard usually takes 3-4 hours. I do not staple the closure.
(2) Medicare patients due to their age are at higher risk for wound healing problems.
(3) 90 day global fee period – These patients routinely need follow-up care and that care is not billable.
Recently I ignored my better judgment and performed the operation for a lady in whom back pain (ICD-9 724.5) and back surgery had been long term problems. She also had a pretty nasty rash (ICD-9 692.89 Dermatitis and eczema [in the infra-mammary fold]) under her right breast that just wouldn’t go away. These of course were all in addition to the usual diagnosis of large breasts (ICD-9 611.1 Hypertrophy of breast.)
Medicare showed me yet another reason for my hesitation to do these cases when they denied payment for the operation saying it was not medically indicated. They will probably pay on appeal, but the thought that I should have to appeal the case adds insult to injury.
