I just got back from a wonderful week in Toronto, Canada. No, I wasn’t up there to take tips on how to impose socialized medicine on an unsuspecting public, notwithstanding what some of you may incorrectly-surmise about my political leanings.
Rather, I was there to attend ACP’s annual scientific meeting, during which I had the opportunity to serve as faculty for three separate scientific sessions that discussed the impact of the new Patient Protection and Affordable Care Act (PPACPA) of 2010 on internists and their patients. Several hundred ACP members attended these sessions.
And guess what? Rather than encountering doctors who were angry at the new law and ACP’s support for it, I instead found an engaged and curious group of internists who are looking at health reform in a reasoned, measured and open-minded way. Read more »

This blog was written from Toronto, Ontario, where ACP’s elected Board of Governors met to provide direction on the policies to be advocated by the organization.
One issue raised by many of the governors is the enormous economic pressure on smaller internal medicine practices, and what the ACP might be able to do about it. Today, most physicians work in private practices of ten or fewer. Read more »

Once again, Congress is playing with fire by not enacting a permanent solution to the Medicare SGR (sustainable growth rate) physician payment cut problem.
Congress got itself tied up in knots trying to figure out a way to reverse a 21% cut in Medicare payments to doctors that went into effect yesterday. It ended up agreeing to legislation, which was signed into law late Thursday evening by President Obama, to restore payments to the pre-cut (2009) levels through the end of May.
The action, though, may have come a dollar short and day late. CMS has indicated that it had no choice but to tell carriers to begin processing claims with the 21% cut, starting yesterday. Read more »

You may have noticed, uncharacteristically for me, that I haven’t posted a blog in week. I thought it would be better to allow the readers to post their own reflections, and you did — with comments ranging for unabashed pride to skepticism to disdain for the law and the American College of Physician’s (ACP’s) role in bringing in about.
I respect the principled arguments made by those who believe that the legislation gives the government too much control or those who fear that it will add to the deficit and public debt, even though the Congressional Budget Office (CBO) says otherwise. But there is one claim made by some of the critics that sticks in my craw, which is that the legislation will result in “massive cuts” to Medicare. Here are the facts. Read more »

With the vote on the healthcare reform legislation behind us, it’s nice to believe that we can now move to the stage where people begin looking at what the legislation will and will not do — not based on speculation or the political rhetoric, but what is actually in the legislation itself.
I realize that this is unlikely, since we all tend to engage in cognitive dissonance when confronted with information that does not square with our own pre-conceived notions, political leanings, and philosophical bent. I know I do it (as much as I try not to) and I’m sure this is true of just about all of us. Still, there are trusted and highly-credible sources of information that I hope will be of value to anyone who is open to learning more about the new healthcare legislation and its potential impact.
