As a hospitalist physician of seven years and taking care of dialysis patients, I’ve come to the conclusion that a dialysis survival gene exists. I talked with a nephrologist the other day about dialysis survival. Here’s what he said:
“If you take all dialysis comers, every year 25% of them will die.”
There is a broad range of dialysis survival. A 94-year-old with severe COPD, CHF, and dementia will not have the same survival statistics as a healthy 27-year-old with acute interstitial nephritis. The protoplasm from which you begin with often times determines the dialysis survival.
There are many factors that determine dialysis survival statistics. Some of them include, age, race, weight, and even the length of the dialysis treatments. But no where have I seen reported the association of dialysis survival with Happy’s presumed dialysis surivival gene. Read more »

Name: “21” (% to be cut from Medicare)
Protagonist: Dr. Rob and a cast of thousands of physicians (Kiefer Sutherland wouldn’t work for such small payment.)
Villain: Evil SGR (Sustainable Growth Rate) conspiracy to cut Medicare by 21% across the board.
Victim: The elderly population depending on Medicare for payment of their medical care.
Plot: A follow-up to the popular drama “Lost” where members of congress were stranded in Washington D.C. with the task of reforming healthcare without any contact or communication from doctors and patients. This new drama “21″ tells the tragic tale of an industry under siege and a population facing possible disaster.
Already stretched to the limit by the paltry reimbursement from Medicare for primary care office visits, Dr. Rob and his band of physicians is hit by the evil conspiracy of SGR, a secret society whose goal is to harm the elderly people in the country by driving away all people willing to give them care. The congress, tired out from haggling over the healthcare reform bill, allows evil SGR to exert its power in the name of “fiscal responsibility.” Read more »

“Am I having a heart attack?” From the files of the strange and unusual comes this self-test kit for myocardial infarction, otherwise known as a heart attack.
For the lay people out there, a heart attack occurs when bloodflow to the heart muscle stops, usually because of a blockage in the arteries around the heart (coronary arteries). (It’s what happened to Bill Clinton, although his heart muscle didn’t die, as it likely had collateral bloodflow from other arteries.) When bloodflow stops, the heart muscle dies. When the heart muscle cells die, they release compounds into the bloodstream which can then be detected on blood draws.
That is the basis for detecting a heart attack by drawing blood. There are some compounds that are specific to the heart, such as troponins that will only go up when the heart muscle is dying. Other enzymes, such as the CK go up with any muscle damage, including the heart.
That is the basis of this new self-test kit for heart attack testing from China Sky One Medical that tries to answer the “Am I having a heart attack?” question at home. It was approved in China in 2007 and recently received European Union clearance as well. Read more »

There’s been some buzz on Twitter that the Democrats in the Senate have killed an amendment which would have prohibited federal funding for erectile dysfunction (ED) medications for sex offenders, which means in the upside-down logic of Washington D.C. that the Democrats favor giving Viagra to pedophiles. Right? Well, not exactly.
First, just a point of procedure: Remember, healthcare reform (HCR) is now a law, on the books, signed by the President. But there was this reconciliation sidecar bill which was meant to fix the HCR law as originally passed—it was passed in the House pretty much as soon as the Senate bill was passed. But it had to pass the Senate in the identical form, word for word, in order to go directly to Obama’s desk for signing. If the Senate mucked things up by tacking on amendments, it would have to go back to the House for approval, and given the razor-thin margin in the House, Democrats very much wanted to avoid that.
