Who do you think is likely to be a better doctor: A board certified graduate of one of the top medical schools in America, or a non-certified doctor trained in a foreign country?
If your answer is “I have absolutely no idea,” then you’re probably spending a lot of time looking at the “report cards” that pass for measures of health care quality. And you’re probably confused.
Researchers in Pittsburgh studied 124 process-based quality measures in 30 clinical areas. These process measures are the state-of-the-art ways in which government and private insurers are checking up on the quality of medical care. They include things like making sure patients with heart problems are prescribed aspirin, and that women get Pap smears. The researchers compared these measures against other, simpler measures, like medical education, board certification, malpractice claim payments, and disciplinary actions.

The first-year medical students I precept were too young to see Tom Cruise’s alter ego Lieutenant Pete “Maverick” Mitchell grace the big screen in the 1986 blockbuster film “Top Gun.” Yet, the story has a relevant analogy to medicine.
According to the film, during the Vietnam war American pilots were relying too much on technology to bring enemy fighters down. They weren’t as skilled in taking out the opposition. They fired their technologically advanced missiles to try and get the job done. They didn’t think. It didn’t work. They forgot the art of dogfighting.
The military discovered that technology alone wasn’t going to get the job done. The best fighter pilots needed the skills, insight, and wisdom on when to use technology and when not to. As a result, the Navy Fighter Weapons School, known simply as Top Gun, was created to retrain the military pilots on this vital lost skill. The goal of the program was specifically to make the best of the best even better.
Like the military, the country is discovering that the healthcare system enabled with dazzling technology isn’t getting the job done either. Read more »

The vision behind ConAgra’s programs is simple: Employees have to be responsible for managing their own care. But, says Charlie, this is easy to say, harder to do. “So we do as much as we can to make it as easy for people to do the right thing.” ConAgra gives its employees a significant financial stake in their well-being, through a health plan that has a $1,500 deductible. ConAgra supplements the plan with a health savings account (HSA) that lets workers use pre-tax dollars to pay for the deductible. I’m speaking [today] at the 23rd Annual Benefits Forum and Expo. This is one of the premier events in the health care benefits industry, and it’s a thrill for me to be the opening speaker on the “Health Care” track.
I’m presenting along with Charlie Salter, the VP of Benefits of ConAgra, one of our customers at Best Doctors. The talk Charlie and I will give is called “Real Results: When Individuals are in Control of their Health Care.”
As regular readers know, good things happen when people are in control of their care. They have a chance to make sure they’re not one of the 20 percent of people that end up with an incorrect diagnosis, or the more than 60 percent of people that end up with the wrong treatment. I have a friend who had a blocked cardiac artery. A couple of years ago he had angioplasty on it, and his doctor inserted stents. The stents got rid of his chest pain and other symptoms, but didn’t do anything to get at the underlying cause of the blockage, which had to do with an unhappy combination of genetics and a –- perfectly admirable –- taste for rich, fatty foods. Like steak. (More on that in a moment.)
Before having the procedure, his doctors spent a lot of time with him explaining what the surgery would and wouldn’t do. In particular, the doctors explained that the stents would do their job, but he had to do his. He needed to eat better, exercise more, and take his medications. He’s followed most of that advice, and is doing well.
Unfortunately, his experience is not typical. A recently published study found that more than 80 percent of patients who had gotten angioplasty and stents thought they were alone a cure for their problems.
These patients are wrong. So how can it be that they are coming to this strange conclusion? According to some, it’s the doctors’ fault. Read more »
