Last week someone posted on Twitter that they had swallowed a plastic toothpick. What to do? So they turned to the hive for help. “What should I do?” I thought as I read my Twitter feed. I was paralyzed in a way. I wanted to share my experience with hundreds of patients had swallowed pins, toothpicks and other pointy things. I specialize in just this sort of thing. But short of a random comment about gastric emptying, I kept to myself. Why? Because once I lend a hand I’m all in.
The simple offer of patient-specific advice constitutes a relationship in the eyes of the law. Once involved, I potentially share responsibility in whatever happens to someone. Crazy but true. It’s just a matter of time before slip-and-fall lawyers hold physicians accountable for helping out in the social sphere.

A little over a year ago, before Twitter was the tech/pop culture phenomenon it is today, doctors like myself had a problem: how do you identify other health professionals on Twitter? (At the time, there must have been at least dozens. Dozens.)
This was the first solution. In retrospect, it was hilariously cobbled-together:
This is a feed containing the conversations of all known doctors and medical students who use Twitter: http://feeds.feedburner.com/doctorsontwitter. (If that doesn’t work, you can try the original feed from Yahoo Pipes instead.) Technical details, for those interested: I used this list of doctors/medical students on Kidney Notes, ran each person’s Twitter feed through Yahoo Pipes, then burned a FeedBurner feed.
When FriendFeed debuted, I created “The Doctor’s Room,” which was populated by both Twitter feeds and RSS feeds of physicians. Unfortunately, the “room” feature was poorly designed by FriendFeed (which has since been acquired by Facebook). Like the Yahoo Pipes experiment, the FriendFeed room was an educational failure. The call schedule is hidden somewhere inside!
Dictation:
‘The patient was seen in the emergency department by Dr. Niemans, who is the acting hospitalist on call for Dr. Whitman’s group, who usually admit for the patient’s actual primary care provider, Emily Knight, PA, who works for Dr. Robelo, who no longer takes call, but admits his patients to the hospitalist.
Because the patient has COPD and an acute left-lower-lobe pneumonia, pulmonology was consulted. However, no pulmonologist is available this weekend. Pulmonary is being covered by Dr. Albertson, pulmonologist in the neighboring city. I spoke with Dr. Albertson about this and he told me he wasn’t taking call for our patients, and why did people keep bothering him.
The patient’s cardiologist, Dr. Rease, is being covered by Dr. James. I spoke to Edgar, PA for Dr. James who said that as of 7am, Dr. James was trading call with Dr. Housefield while Dr. James went to his son’s soccer game, but if I had any questions I should call Dr. Housefield’s Nurse Practitioner Michael, who would be rounding for Dr. Housefield, Dr. James and Dr. Josefson, at least until soccer was over or something bad happened. Some physicians may be hesitant to participate in social media outlets, like Facebook and Twitter.
Well, get over it.
Great post by pediatrician Bryan Vartabedian who addresses this topic. Indeed, physicians have lost control of the online message, especially with, according to recent data, 60+ percent of patients visiting the web first when looking for health information.
Instead, anti-vaccine proponents and homeopaths have embraced the Internet, and now exert tremendous influence on patients. We doctors have no one to blame but ourselves for being so slow to get online. Read more »

I had an interesting visit with the husband of my niece last evening. He works as an ER doctor that is self-insured group of 60 physicians that cover the ER needs of four hospitals in Clark County near Las Vegas.
What is interesting is they are self-insured to save costs. As a group, then, they know how much per patient they must collect to assure liability care for every patient that comes to their emergency rooms.
That amount is $17 per patient per visit.
Guess how much their group receives for care they render to a Medicaid patient for a “level two” visit (minor problem: ear ache, sore throat, etc.)
