Doctor Suing For Bad Ratings Online

Dr. Kimberly HenryI must say I think Dr. Kimberly Henry, cosmetic surgeon, has made a big professional mistake. She has filed a lawsuit to stop online reviewers from badmouthing her on the Internet. She is seeking injunctions against at least 12 reviewers from sites such as Yelp.com and DoctorScorecard.com. Dr. Henry claims libel and defamation, invasion of privacy and interference with prospective economic advantage and is seeking $1million in general damages and $1million in special damages, etc.

Now I don’t know Dr. Henry nor do I know of her plastic surgery technique. I don’t know who the disgruntled patients are or if they are unfairly targeting her. What I do know is that the Internet is here to stay and there’s no place to hide if you don’t provide excellent customer service. Medical Receptionist Extinction?

Medical receptionists beware — your days are numbered.

This little gizmo was placed in one of our facility’s lobbies this week. (No, it’s not being used to get your boarding pass at the airport, but it’s amazing the parallels healthcare is taking with the airline industry.)

Instead, it’s used to check in patients presenting to have their blood drawn for prothromin times. Just swipe your credit card, confirm your appointment, sign your name, and away you go!

On seeing this, one doctor exclaimed: “But INR checks are my patients’ only chance to get out and socialize!”

Fortunately for now there are still human assistants there to help patients learn how to use the new device.

How An Empowered Patient Finds A New Doctor

What if the average patient (person) knew what healthcare insiders, providers and expert patients know?

Take the process of looking for a new personal physician. Conventional wisdom tells people that when looking for a new physician they need to consider things like specialty, board certification, years in practice, and geographic proximity. Online services like Health Grades allow you to see and compare the satisfaction scores for prospective physician candidates.

But industry insiders know different. Consider those patient satisfaction scores for physicians. In reality, “one can assume that the quality of care is actually worse than surveys of patient satisfaction would seem to show,” according to a 1991 lecture by Avedis Donabedian, M.D.:

“Often patients are, in fact, overly patient; they put up with unnecessary discomforts and grant their doctors the benefit of every doubt, until deficiencies in care are too manifest to be overlooked.”

Given the constant drumbeat about the lack of care coordination and medical errors, it would seem that some people (patients) are beginning to reach the breaking point alluded to by Dr. Donabedian. The empowered among us are starting to compare physicians (and the hospitals that employ them) to a higher standard — a higher standard that reflects the nature and quality of the medical services physicians actually provide. Internists, I expect, will identify with Edward Bear.

Richard Baron’s study in the NEJM on the amount of work he and his colleagues do outside of an office visit — the “bump, bump, bump” of a busy internal medicine (IM) practice — has resonated with many of his colleagues.

Jay Larson, who often posts comments on this blog, did a similar analysis for his general IM practice in Montana, and found that for every one patient seen in the office, tasks are done for 6 other unscheduled patients. Jay writes: “So really there [are] internists [who] are managing about 130 patients per day. Not much consolation when they only get paid for 18 per day.” Read more »

Faith In Healthcare Is Falling

A newly-created index of consumer healthcare confidence has fallen steadily this year, reports The Thomson Reuters Consumer Healthcare Sentiment Index. Consumers report declining confidence in their ability to access, use, and pay for healthcare. The index, set at a baseline of 100 in December 2009, is now at 97.

More consumers reported difficulty paying for services and insurance, or reported a reduction or cancellation of their insurance. More delayed or failed to fill a prescription in the past three months or canceled a diagnostic test (such as blood work, X-ray or mammogram). Further, consumers expect the situation to worsen in the next three months, including putting off elective surgery.

Thomson will report figures monthly and has published their methodology online.
